After knee replacement surgery, your body is already managing several things at once: healing the surgical wound, managing post-operative inflammation, rebuilding muscle strength, and adapting to the new implant. Summer heat adds a few additional challenges:
This is non-negotiable for the first 4–6 weeks after surgery. Keep your recovery room cool — ideally between 22–24°C. This directly reduces swelling, improves your comfort, makes physiotherapy exercises easier, and protects the wound from sweating. If you don't have AC, at least use a fan with a wet cloth nearby to keep the room temperature manageable.
After knee replacement, you are prescribed blood-thinning medications to prevent clots. These work best when you are well-hydrated. In summer, you need to drink significantly more water than usual — aim for at least 2.5 to 3 litres of water per day. Include ORS (oral rehydration salts) if you're sweating heavily, and coconut water is an excellent natural hydration option.
Applying ice to the operated knee for 15–20 minutes every 2–3 hours is standard practice after knee replacement. In summer, this is even more important. Ice helps control swelling, reduces pain, and keeps the knee comfortable despite the ambient heat. Use a cloth between the ice pack and skin — never apply ice directly.
The surgical wound needs to stay clean and dry. In summer, perspiration around the knee can increase infection risk. Follow these wound care guidelines:
Keeping your operated leg elevated (higher than heart level) helps drain excess fluid and significantly reduces swelling. In summer this is doubly important because heat increases fluid retention in the leg. Use a firm pillow stack or a wedge cushion to keep the leg elevated comfortably.
Don't skip physiotherapy exercises because of the heat. However, schedule them for the coolest parts of the day — early morning (before 9 AM) or evening (after 7 PM). Exercise in an air-conditioned room whenever possible. Staying consistent with your rehabilitation schedule is critical for a good recovery regardless of season.
Wear breathable cotton clothing around the knee. Tight socks or compression garments in extreme heat can be uncomfortable but are sometimes still necessary for swelling and clot prevention — your physiotherapist will advise based on your specific situation.

There is no universally "best" season for knee replacement. Winter is popular because recovery is more comfortable and physiotherapy walks are easier in cooler weather.
